Sleep apnea testing*

Sleep apnea is detrimental to weight loss, because it disrupts hormones, slows metabolism, and causes fatigue that prevents you from increasing physical activity. Sleep apnea is also associated with high blood pressure, heart attacks, strokes, diabetes, and death.

Cortisol testing**

If you do not achieve at least 5% weight loss within the first 3 months of treatment, there could be an underlying medical issue preventing weight loss. The same concept applies when the medication stops working despite increase in dose and continued lifestyle changes. Oftentimes cortisol "the stress hormone" is to blame.

Who this program is for — and who it is not for

This program is designed for adult women with hormone-related weight concerns: thyroid disease, PCOS, perimenopause and menopause, particularly when diet, exercise and previous weight loss medication have not worked.

Whether you are a candidate is decided at your evaluation, not on a website. GLP-1 medication is not appropriate for everyone. It should not be used by people with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2, and it is not used in pregnancy or while trying to become pregnant. A history of pancreatitis, gallbladder disease, severe gastrointestinal disease, certain eating disorders, or specific diabetes medications all change the picture. That is what the history, exam and labs are for.

What your evaluation covers

An in-office visit and urine drug screen are required before medication in this program is prescribed. Labs include CBC, CMP, A1C, TSH and a lipid panel, repeated every three months. Sleep apnea testing happens in the first month through a home sleep test, and one-time cortisol testing in month four. Testing is selected according to your history, symptoms and examination; no test is guaranteed to identify a cause of limited weight loss.

About the medication

When medication is appropriate, FDA-approved options are discussed first. A compounded medication may be considered only when it is lawful, clinically appropriate for you, and prescribed to meet a documented patient-specific need.

Compounded GLP-1 products are custom-compounded, vary from pharmacy to pharmacy, have not been evaluated or approved by the FDA for safety, efficacy or quality, and require a valid prescription from a licensed healthcare provider after an individual medical evaluation. They are not generic versions of brand-name medications such as Ozempic, Wegovy, Mounjaro or Zepbound. Enrolling in a program does not guarantee that any medication will be prescribed, compounded, available or appropriate for you.

Risks and side effects

Common adverse effects include nausea, vomiting, diarrhea, constipation and abdominal symptoms. Their severity and duration vary — contact the clinic for persistent or severe symptoms. More serious risks reported with these medications include pancreatitis, gallbladder disease, kidney injury from dehydration, and low blood sugar when combined with certain diabetes medications.

FDA-approved semaglutide and tirzepatide products carry boxed warnings concerning thyroid C-cell tumors observed in rodents; it is not known whether they cause these tumors in humans. The exact warnings depend on the product prescribed.

What happens if progress is slower than expected

Weight-loss results vary and no specific amount or timeline can be guaranteed. If progress is less than expected, Carolyn will reassess treatment response, adherence, side effects, dosing, lifestyle factors and other medications, and whether your symptoms or examination support evaluation for another condition.
